The total cost is $375 The fee includes the clinical interview, test administration, scoring, interpretation, report writing, and an optional feedback session.
Next Steps After the Evaluation
- The assessment results will be processed within one week.
- Once completed, the report will be sent directly to your surgeon—please ensure a signed consent form is on file.
- The report will highlight strengths, potential concerns, and recommendations to support your post-surgical recovery. Your surgeon will use this information to determine whether to proceed with surgery.
Follow-Up & Support
- If significant concerns arise, a feedback session will be scheduled to review the results.
- Recommendations may include mental health treatment or other interventions before surgery.
- Feedback sessions are available upon request, and documentation will be provided to your surgeon.
- In addition to the pre-surgical assessment, Tywanda Williams, LCSW, BCBC (Board Certified Bariatric Counselor) offers individual therapy before and after surgery to support lifestyle adjustments, develop healthy coping strategies, and monitor mental well-being.

The fee for the assessment is $375. Insurance is not accepted for the assessment as insurance does not cover the total assessment cost for an LCSW.
